Family doctor plan*
Adult
$1875/Yearly
- Priority emergency visits reserved for members during regular business hours
- Medical consultations during regular clinic hours
- Limited to 12 medical visits per year
Child (17 years & younger)
$1500/Yearly
- Priority emergency visits reserved for members during regular business hours
- Medical consultations during regular clinic hours
- Limited to 12 medical visits per year
*The family doctor plan does not include the fees related to blood tests or accessory fees as detailed in the patient brochure.
N.B. The plan is for a one year period and automatically renews unless we receive a written request not to renew one month before its expiry.

Note
Appointments with consultants (exercise stress test, vasectomy and women’s health) requires a deposit by credit card. In case of cancellation less than 48 hours, the amount will be charged without reimbursement.
